CVOR Registered Nurse Travel/Contract Position
Location: Flagstaff, Arizona
Contract Length: 13 weeks
Job Details
- Specialty: Registered Nurse (CVOR)
- Shift: Day shift, 4x10-hour shifts (6:30 AM – 5:00 PM)
- Hours: 40 hours per week
- On-Call: Yes, 18 days of call within an 8-week period
- Weekend Requirement: As needed
- Floating: Must be willing to float to Main OR when no CVOR cases are scheduled
- Experience Required: Minimum 2 years of OR experience, including CVOR and vascular coverage
- First-Time Travelers: Accepted
- Locals: Accepted
- Guaranteed Hours: 40 hours/week; facility may call off up to 6 times during the 13-week assignment
- Blocked Scheduling: No
- RTO Restrictions: Unit manager approval required
- Additional Responsibilities: Willingness to help train core staff and assist where needed

Required Qualifications
- Valid RN license in Arizona
- Minimum 2 years of CVOR experience, including experience within the last year
- ACLS certification issued by the American Heart Association
- BLS certification issued by the American Heart Association
- Associate of Science in Nursing or Associate Degree in Nursing
- Operating Room (OR) experience
Required Documentation & Compliance
- Valid Driver’s License or State ID (passport not accepted alone)
- Background checks including 7-year criminal search, SSN trace, sex offender search, EPLS/GSA/SAM, and HHS/OIG within 30 days of start
- Physical exam within 1 year confirming ability to perform essential job functions
- Fit mask (N95) test within 1 year and updated annually; facility may provide onsite fit testing if needed
- Immunizations and titers:
- Tetanus/Diphtheria/Tdap (within last 10 years or declination)
- Negative TB test or chest X-ray within 1 year, updated annually
- Measles, Mumps, Rubella (positive titer or proof of 2 MMR vaccines)
- Hepatitis B (positive titer, 3-dose series, or declination)
- Varicella (positive titer or proof of 2 vaccines)
- Drug screening: 10-panel urine test within 30 days of start, updated annually
- COVID vaccination series or declination with facility-specific form
- Seasonal flu vaccination or declination with facility-specific form; mask required if declined
- Completion and signing of facility-specific policies including confidentiality, substance abuse screening, professional attire, and security questions prior to start
- Facility-specific job description form must be completed prior to start
- Two professional references required (one from most recent employer within 1 year, second within 2 years)
- Employment application covering last 3 years with documented gaps over 30 days
Additional Notes
- Radiation badge required for CVOR RN role; related forms must be completed within 1 week of contract signing
- Candidate must be willing to assist with training and support of core staff
- Facility expects adherence to all safety and quality standards
