Hospice Nurse

Overview

• Contract role in Sunnyvale, California
• Day shift: 8:30am–5pm
• 40 hours per week
• Schedule can be flexible based on patient availability
• One weekend per month

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Hospice experience
• Case Management (1 year of experience)
• Case Management experience within the last year

Role Summary

• Admits patients to hospice service
• Assesses patient and family physical, psychosocial, environmental, safety, and developmental needs
• Develops individualized plans of care
• Documents observations and interventions related to the patient’s environment
• Coordinates care with a multidisciplinary team
• Participates in quality assessment and performance improvement activities
• Conducts follow-up visits as needed

• San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities as needed
