Required Qualifications

• Valid Occupational Therapist license in Texas
• Occupational Therapist (1 year of experience)
• Occupational Therapist experience within the last year
• NBCOT
• Experience in a Long Term Care or Rehabilitation setting

Job Responsibilities

• Screens and evaluates patients with cognitive and/or ADL physical functioning disorders and develops a plan of care
• Uses standardized assessments and evidence-based practice
• Provides rehabilitative treatment interventions, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and supportive or assistive devices
• Assesses progress, adjusts treatment, evaluates outcomes, and makes referrals as needed
• Provides consultation and counseling to patients, families, caregivers, and other service providers
• Communicates with the supervisor and other health team members about patient progress, barriers, and treatment plans
• Provides discharge summaries
• Documents required information in the electronic documentation system and maintains patient records
• Records treatment minutes
• Participates in required meetings, trainings, in-services, and conferences
• Obtains necessary DME or supplies
• Supervises and supports occupational therapist assistants, rehabilitation aides, and/or students

Additional Information

• 8 hours per day
• Must be submitted at the posted rate; higher requested rates may be redirected
