Job Opportunity: RN ICU - Nashville, Tennessee

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Start Date: December 8 (Non-negotiable)
Shift Hours: 36 or 48 hours per week
Location: Floating between units/facilities within a 35-mile radius based on staffing needs, patient census, and acuity. Notification provided before scheduled shift if floating is required.

Required Credentials & Certifications

Licenses & Verification (Required for Submission)

• Nursys Verification: Must be completed within 30 days prior to start. If any disciplinary actions, complete FACIS Questionnaire.
• License/Certificate Verification: Current license and primary source verification within 30 days prior to start.
• Driver's License/State ID: Must be valid, not expired, and match address on file.
• Professional Profile/Resume: Required for submission.
• Two Professional References: At least two qualifying references prior to first assignment.

Certifications (Required for Onboarding)

• BLS (AHA)
• ACLS (AHA)
• NIH Certification: Group A updated yearly; Groups B-F every 2 years.

Health & Immunizations (Required for Onboarding)

• MMR: Proof of two vaccines or positive IGG titer. No declinations accepted.
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider. No declinations accepted.
• Tdap: Valid within 10 years. No declinations accepted.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted with mask requirement.
• Fit Test: Required for all travelers; preferred before start, can be completed onsite.
• 2 Step PPD or IGRA: Recent testing required with annual TB questionnaire updates.
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
• Ascension Color Vision Test: Completed prior to start and updated every 3 years.

Testing & Competencies (Required for Onboarding)

• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics. Completed within a year of start, updated yearly. Minimum 80% score, max 2 attempts.
• Ascension EKG Exam: Passing score of at least 80%, max 2 attempts. Must be updated to latest version prior to clearance.
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation.
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start; updated annually for specialty.
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Includes testing for 12 specified drugs with MRO review if applicable.

Additional Onboarding Requirements (Required for Onboarding)

• Ascension Onboarding Requirements Education Attestation: Completion of pre-hire 6-hour training.
• Ascension Nursing Contract Worker Onboarding: Completed onsite.
• Ascension Nova StatStrip: Pre-hire modules for non-allied positions.
• Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check, updated annually.
• Ascension EPLS: Must be run within 90 days of start and rerun if break in service > 90 days.
• Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start.
• Ascension Employment Verification: Third-party verification for past 7 years, updated annually.
• Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.

Optional

• Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
• Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended if applicable.
• Hiring Manager Evaluation: Post-assignment evaluation.
• Ascension Name Change Document: Legal documentation of name change.
