Job Opportunity: Case Manager (Registered Nurse)

Location: Burlingame, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

Cross-trained Case Manager RN to float and work as needed across various hospital units
Acute Care experience required
Direct hospital-based discharge planning experience to the community (not utilization management focused)
Strong inpatient and Emergency Department case management experience
Knowledge of California insurance requirements and county regulations related to complex discharges
Ability to function independently in an acute care hospital setting

Shift Details

Variable start times between 7:00 AM and 3:30 PM
Every other weekend required
Floating to various units as needed
Typical shifts: Day (7:00 AM - 3:30 PM) or Evening (3:00 PM - 11:30 PM)
8 hours per day, 40 hours per week

Required Qualifications

Valid Registered Nurse (RN) license in California
Minimum of 5 years of Case Management experience, including experience within the last year
Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Experience in Discharge Planning
Knowledge of insurance requirements related to case management
Acute Care experience
Emergency Department (ED) experience
