Registered Nurse – Transfer Center Clinical

Location: Rancho Cordova, California
Contract Length: 13 weeks
Shift: Nights
Hours: 12 hours per day, 36 hours per week
Weekend Requirement: Every other weekend
Holiday Requirement: Must be available to work holidays if needed

Job Overview

• Facilitate patient transfers (any patient type) from hospitals within and outside the system.
• Manage cases after intake, including patient assessments, vitals, and clinical information.
• Identify and communicate with accepting physicians and specialists to secure patient placement.
• LVNs prioritize referring patients to external facilities if internal placement is unavailable.
• No direct hands-on patient care involved.
• Manage multiple cases simultaneously (10-15 cases at a time on average).
• Utilize multiple computer applications including Cerner, TeleTracking, Tiger Connect (texting MDs/specialists), and Google apps for communication and documentation.

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Minimum 1 year of Long Term Acute Care experience within the last year
• Emergency Department (ED) experience
• Intensive Care Unit (ICU) experience
• Acute Care experience
• Experience with Cerner electronic medical records
• ACLS issued by the American Heart Association
• BLS issued by the American Heart Association
• NIHSS certification

Certifications

• ACLS (AHA)
• BLS (AHA)
• NIHSS

Orientation & Training

• 1 day general hospital orientation.
• 3-5 days department-specific orientation/training, which may occur on any shift.

Scheduling & Work Environment

• Must be available for scheduling any day of the week.
• Limited to no remote work (Return to Office required).
• Scrubs of any color allowed if clean and professional.
• Casual attire allowed: clean jeans (no rips/tears), clean hoodies without large writing.
• Free onsite parking and EV charging available.

Additional Requirements

• Two supervisory references required: one within the last 12 months and one within the last 3 years.
• Candidates must not have been employed full-time, part-time, or PRN by the system or its affiliates within the past year (PRN through outside firms is acceptable).
• Must pass a dyslexia exam prior to acceptance (two attempts allowed).
• Typing skills assessment recommended to demonstrate proficiency.

Systems & Tools

• Bed management via TeleTracking.
• EMR systems: Cerner, TeleTracking, Google applications.
• Communication tools: Tiger Connect for messaging physicians and specialists.

Managerial Support

• Supervisors and leadership team available for support.

Notes

• This position involves no direct patient care.
• Candidates without prior travel experience are accepted.
• Must be able to work 12-hour night shifts with every other weekend coverage.
