Pharmacist Outpatient - Travel/Contract Position
Location: Kingman, Arizona
Contract Length: 13 weeks
Job Details
- Employment Type: Travel/Contract, full-time
- Shift: Variable between 8:00 AM to 8:00 PM (Day shift typically 8:00 AM to 4:00 PM, 5 days a week, 40 hours/week)
- Weekend work required
- On-call: Not required
- Flexible scheduling possible, but preference for minimal schedule restrictions
- First-time travelers accepted
Required Qualifications
- Minimum 5 years of ambulatory pharmacist experience, including experience within the last year
- Graduate of a Doctor of Pharmacy (PharmD) program
- Basic Life Support (BLS) certification required
- Pharmacist experience
Compliance and Documentation Requirements
- Background checks including 7-year search within 30 days of start, updated annually (SS trace, county searches, nationwide, sex offender, education verification)
- Annual EPLS/GSA/SAM and HHS/OIG checks
- Transcript/Diploma and education verification through primary source
- Medication/Pharmacology competency exam as applicable
- Physical exam within 1 year of start
- Negative TB test or chest X-ray within 1 year, with annual TB questionnaire
- Immunization or titer proof for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 30 days of start (instant drug screen acceptable)
- Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
- Two professional references within 3 years
- Annual skills checklist
Additional Notes
- Clinician interview required
- Locals and first-time travelers are compensated equally
- Facility-specific forms and agreements must be completed as a priority
- Clear, passport-style photo with solid background required for badge issuance
