Job Opportunity: RN Hospice Case Manager

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 20 weeks
Work Schedule: Monday - Friday, 36 hours per week

Job Description

  • Provide hospice care in a home setting as an experienced Hospice RN.
  • Manage hospice cases effectively.
  • Prior experience with rural travel is preferred.
  • Experience with McKesson/Netsmart EMR systems is preferred.
  • Preference for candidates with no Return to Office (RTO) requests for this contract.

Required Qualifications

  • Valid RN license in Vermont
  • Minimum of 1 year of case management experience, including within the last year
  • BLS certification
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Experience in a Home Health setting
  • Hospice nursing experience

Additional Information

  • This position involves travel to rural areas.
  • Work schedule is consistent Monday through Friday.
