Registered Nurse (Medical/Surgical/Telemetry) - Long-Term Assignment

Location: Ithaca, New York
Contract Length: 13 weeks
Shift: 6:45 PM – 7:15 AM (Night Shift)
Employment Type: Long-Term Assignment (LTA)

Job Description

• Bedside patient care position
• Requires experience with Epic electronic health record system
• Work schedule includes every other weekend and every other holiday

Required Qualifications

• Valid RN license in New York
• Minimum 2 years of Medical/Surgical and Telemetry nursing experience
• Medical/Surgical and Telemetry nursing experience within the last year
• Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
• Advanced Cardiovascular Life Support (ACLS) certification issued by American Heart Association or American Red Cross
• NIH Stroke Scale (NIHSS) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Valid driver’s license
• Experience with Epic electronic health record system

Health & Safety Requirements

• COVID-19 vaccine proof or agency declination (optional)
• MMR: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of first dose
• Varicella: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of first dose
• Tdap: Within last 10 years or agency-specific declination form
• Hepatitis B: Positive antibody titer, 3 vaccine series, or agency-specific declination form required
• Tuberculosis (TB) testing: One of the following required and must be current within 12 months:

  • Two PPD skin tests (one within 12 months, one within 3 months) or
  • One Quantiferon/Spot test within 12 months
  • For past positive TB tests, chest X-ray within 12 months may be required
• Fit Test for respiratory protection within last 12 months, must be completed prior to start
• Physical/Health Statement within past 12 months confirming no restrictions, including height, weight, vitals, and vision screening

Documentation & Onboarding Requirements

• State license verification completed within 30 days prior to start
• Valid US driver’s license or state ID (not expired)
• Agency background check within 30 days of start, including:

  • SSN address trace report covering last 7 years
  • County criminal searches for all counties lived/worked in last 7 years
  • National criminal check and FACIS 3 check
• 9+ panel drug screen excluding marijuana (per New York state regulations)
• Centralus resume with full employment and education history for last 7 years, including gaps
• Two professional references:

  • One supervisor reference from last 12 months
  • One supervisor or peer reference from last 3 years
• Verification of most recent employer (agency verification accepted)
• Signed and dated Centralus job description
• Color headshot photo (professional, no filters, JPEG format)
• Completion of Centralus Health Personal Information Form, Code of Conduct, Confirmation of Assignment Form, Self Study Packet and Quiz, Sexual Harassment Acknowledgement, and Parking Registration Form (all found in document library)
• TB Questionnaire Form (document library)
• 7 Instant Background Checks (manual upload required)

Additional Notes

• Must maintain all certifications and health requirements throughout the assignment
• All required documentation must be current and complete prior to start date
