Required Qualifications

• License: Valid Occupational Therapist license in Texas
• Experience: Pediatric Occupational Therapist experience

Additional Requirements

• Unexpired government-issued photo ID
• Background check within 1 year of start, including SS trace and county searches for the last 7 years of locations lived/worked/in school, nationwide sex offender search, and annual updates
• EPLS/GSA/SAM clearance before facility start
• HHS/OIG clearance before facility start
• Primary source verification of state license before facility start and upon expiration
• National certification/verification before facility start and upon expiration, as applicable by profession and state
• Signed acknowledgment of the Joint Commission Training Safety Manual, updated annually
• Competency exam, as applicable to profession/specialty
• Physical exam statement within the last 12 months of start
• Negative TB test or chest X-ray within the required timeframe
• TB questionnaire annually for positive TB and in lieu of annual TB test
• Measles, mumps, rubella, hepatitis B, and varicella documentation as specified in the source
• 9-panel drug screen within the last 12 months of start
• 2 professional references within 3 years of hire
