Required Qualifications

• Valid Physical Therapist license in Arizona
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS issued by American Heart Association

Schedule / Work Expectations

• A weekend day is required
• May be asked to float to surrounding clinics
• First-time travelers are accepted
• Local candidates are accepted

Candidate Preferences

• Generalist with confidence treating medical diagnoses
• Ability to manage moderate to maximum assistance patients
• Comfortable with co-treatments
• Able to work with ventilated patients
• Able to adapt quickly and take additional evaluations as needed

• Unit manager approval is required for requested time off
• Employment application required
• Two professional references required
• Background check required
• Education verification required
• Physical exam required within 1 year of start
• Drug screening required within 30 days of start
• TB screening required within 1 year of start
• COVID vaccination or declination required
• Flu vaccination or declination required
• Tdap documentation required
• Measles, mumps, rubella, varicella, and hepatitis B immunity or documentation required
• Photo for badge required
• Security question form and travel information verification must be completed before start
• Workforce member confidentiality agreement, substance abuse screening policy, professional attire and appearance policy, and unit-specific job description must be reviewed and signed before start
