Required Qualifications

• License: Valid Physical Therapist license in Arizona
• Experience: Physical Therapist (1 year of experience)
• Recent experience: Physical Therapist experience within the last year
• Certification: BLS - Basic Life Support issued by American Heart Association or American Red Cross

• Transcript or diploma
• Education verification
• Physical exam within 1 year of start
• Negative TB test or chest X-ray within 1 year of start

• Hepatitis B documentation
• Varicella immunity documentation
• Drug screen within 30 days of start
• Flu vaccination or declination during flu season
• 2 references within 3 years of start
• Annual skills checklist
