Job Opportunity: Home Health Registered Nurse (RN)

Contract Length: 13 weeks

Job Description

• Provide skilled nursing care to patients in their homes, focusing on diverse populations and geriatric patients.
• Perform comprehensive patient assessments and collaborate with providers to develop and oversee individualized Plans of Care.
• Coordinate interdisciplinary care and certify providers as needed.
• Manage 4-6 patients per day, completing charting onsite using Homecare Homebase (HCHB) and Oasis systems.
• Participate in an on-call rotation, including one night of call during the week and weekends as needed.
• Work schedule: Monday to Friday, 8:00 AM to 5:00 PM (40 hours/week).
• Must be willing to travel up to 50 miles from home base to patient locations.

Required Qualifications

• Valid Registered Nurse (RN) license in Texas.
• Minimum of 1 year of Home Health nursing experience within the last year.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Current BLS (Basic Life Support) certification issued by the American Heart Association or American Red Cross.
• Experience with Homecare Homebase (HCHB) charting system.
• Experience with OASIS documentation system.
• Willingness to travel up to 50 miles from home base to patient locations.

Certifications & Compliance Requirements

• BLS (Basic Life Support) required.
• ACLS (Advanced Cardiac Life Support) recommended (AHA or ARC, RQI accepted).
• PALS (Pediatric Advanced Life Support) if applicable.
• Background checks including 7 years residency/work history, SS trace, county searches, sex offender registry, FACIS Level 1, Texas state driver’s record.
• Annual EPLS/GSA/SAM and HHS/OIG checks.
• Physical exam within 12 months confirming good health.
• Negative TB test or chest X-ray within required timeframes; annual TB questionnaire.
• Immunization or titer proof for Measles, Mumps, Rubella, Varicella, and Hepatitis B declination.
• 9-panel drug screen within 12 months.
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31).
• Annual skills checklist and competency exams including medication/pharmacology and infection control/bloodborne pathogens.
• Valid driver’s license and auto insurance required.

• Dress code: Scrubs, any color.
• Local candidates accepted within a 50-mile radius.
• Mileage reimbursement per IRS standard.
• First-time travel nurses accepted.
• Must sign job description prior to start.
• References required: 2 professional references for travel/allied/per diem acute care, 1 for per diem non-acute care within 3 years.
• Employment application and orientation safety manual acknowledgment required.
