Overview

Schedule / Work Setting
- Every other weekend in the ED
- Float to all inpatient units
Required Qualifications
- Valid RN license in California
- Acute Care Case Management (3 years of experience)
- Acute Care Case Management experience within the last year
- Associate of Science in Nursing / Associate Degree in Nursing
- Experience in a Home Health setting
- Experience in a Long Term Acute Care setting
- Ambulatory experience
- SNF and LTACH referral knowledge
- Discharge planning skills
- Transition of care coordination
- Post-acute placement coordination and services
- Hospice experience
- Skilled Nursing experience
- Experience with Epic
