Case Manager

Overview

Shift: Day, 5x8 hours, 8:00–16:30

Contract length: 13 weeks

Schedule / Work Setting

Every other weekend in the ED
Float to all inpatient units

Required Qualifications

Valid RN license in California
Acute Care Case Management (3 years of experience)
Acute Care Case Management experience within the last year
Associate of Science in Nursing / Associate Degree in Nursing
Experience in a Home Health setting
Experience in a Long Term Acute Care setting
Ambulatory experience
SNF and LTACH referral knowledge
Discharge planning skills
Transition of care coordination
Post-acute placement coordination and services
Hospice experience
Skilled Nursing experience
Experience with Epic
