Physical Therapist - Home Health (Travel/Contract)

Location: Moraine, Ohio
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40 hours


Job Description

  • Full-time position with flexible scheduling; weekend visits optional if clinician chooses.
  • Weekly productivity expectation: 7 patient visits per day (35 visits per week).
  • Patient population: Adults and geriatrics.
  • Geographic coverage includes multiple nearby cities.
  • Home Care Home Base EMR system used; training provided if no prior experience.

Required Qualifications

  • Valid Physical Therapist license in Ohio
  • Minimum of 1 year of Physical Therapist experience, including experience within the last year

Compliance and Documentation Requirements

  • Government-issued photo ID (Driver’s License or State ID).
  • E-Verify completion required prior to start.
  • Legal documentation for any name changes (e.g., marriage license, divorce decree).
  • 7-year criminal background check including SSN trace, national and multi-state checks, and sex offender registry.
  • Drug screening: Negative 9 or 10 panel drug screen including marijuana panel within 12 months prior to documentation due date; lab work must be completed within 72 hours of signing contract.
  • Physical exam within 2 years, signed by MD, DO, NP, PA, or RN, confirming fitness to work and free of communicable diseases.
  • Immunization or immunity documentation for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Varicella
    • Hepatitis B vaccination series, immune titer, or declination (not required for subcontractors)
  • Negative TB test or chest X-ray; if past positive TB, additional documentation and annual TB questionnaire required.
  • Seasonal flu vaccination or approved medical/religious exemption required during flu season.
  • Respiratory safety clearance within 1 year of start, renewed annually.
  • Medication/pharmacology competency exam within 2 years prior to start.
  • Skills checklist updated within 1 year prior to documentation due date and annually thereafter.
  • Two professional references within 2 years of start (one must be a manager/supervisor/charge PT or equivalent). Employment verification accepted if references unavailable.
  • Current resume or employment application required.
