Contract Length

Schedule

8-hour day shift
40 hours per week
Every other weekend required

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
BLS
2 years of Case Management experience
Case Management experience within the last year

Key Responsibilities

Provide individualized nursing care to patients and families
Assess, plan, implement, and evaluate care using the nursing process
Coordinate care with the healthcare team and ancillary staff
Advocate for patients and families
Document assessments, care plans, interventions, and patient responses
Support patient and family education and learning needs
