RN Case Management
Location
La Mesa, California
Contract Length
13 weeks
Schedule
- 8-hour day shift
- 40 hours per week
- Every other weekend required
Required Qualifications
- Valid RN license in California
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS
- 2 years of Case Management experience
- Case Management experience within the last year
Key Responsibilities
- Provide individualized nursing care to patients and families
- Assess, plan, implement, and evaluate care using the nursing process
- Coordinate care with the healthcare team and ancillary staff
- Advocate for patients and families
- Document assessments, care plans, interventions, and patient responses
- Support patient and family education and learning needs
