Cath Lab Registered Nurse - Travel/Contract

Location: Phoenix, Arizona
Employment Type: Travel/Contract
Shift: Day (3 shifts per week, 12 hours each, 7:00 AM - 7:30 PM)
Contract Length: 13 weeks

• Adults and Geriatrics

Required Qualifications

• Valid RN license in Arizona
• Minimum 2 years of Cardiac Cath Lab experience, including experience within the last year
• Current ACLS certification
• Current BLS certification issued by the American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in charge nurse roles

Orientation & Work Environment

• Orientation sessions held bi-weekly
• Scrubs provided
• Weekend shifts not required

Compliance & Documentation Requirements

• Valid government-issued photo ID (Driver’s License, State ID, or Passport)
• Background checks within 1 year, including:

  • Social Security Trace
  • 7-year County and National Criminal checks
  • FACIS III
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
• Education verification (copy of diploma or official transcripts accepted)
• Primary source verification of state license and certifications within 15 days prior to start
• NURSYS verification required within 15 days prior to start
• Physical exam within 1 year, confirming good health and ability to perform job duties (must be completed within 72 hours of signing contract)
• Fit mask test within 1 year, updated annually (can be completed onsite)
• Immunizations and health screenings (must be completed within 72 hours of signing contract):

  • Tetanus/Diphtheria/Tdap (TDAP within 10 years; TD not accepted)
  • Negative TB test (Qgold or Tspot within 1 year; PPD no longer accepted) or chest X-ray if history of positive TB
  • Rubeola (Measles), Mumps, Rubella, Varicella: positive titer or proof of 2 vaccines required; boosters required if titers negative or equivocal
  • Hepatitis B vaccination series or declination within last year or positive IGG titer
• Drug testing with negative results within 30 days of start (specific drug panels required; rapid tests not accepted)
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (Oct 1 - Mar 31)

Additional Requirements

• References per agency requirements (must meet Joint Commission standards)
• Current and updated resume including last 7 years of employment with explanations for gaps over 30 days
• Skills checklist current within 1 year
• Work history verification for past 3 years

• First-time travel nurses are eligible to apply

• Candidate must sign facility-specific documents including confidentiality and job expectations forms
• Candidates must complete an online New Employee Orientation video and upload proof of completion before starting

This position offers a day shift schedule with no weekend requirement but includes on-call duties. The role requires a minimum of 2 years RN experience with prior charge nurse experience and current Arizona licensure. Comprehensive health, background, and compliance documentation must be completed timely to meet facility standards.
