Description
RN Care Manager
Overview
- Location: Chapel Hill, North Carolina
- Setting: Acute care hospital
- Shift: Days
- Hours: 40 hours per week
- Contract length: 13 weeks
Role Summary
Provides ongoing patient assessment, care planning, implementation, and evaluation to support case management, utilization review, and discharge planning.
Required Qualifications
- Valid RN license in North Carolina
- Associate of Science in Nursing / Associate Degree in Nursing
- 2 years of Case Management experience
- Case Management experience within the last year
- Experience in an Acute Care or Hospital setting
- Acute Hospital Case Management experience
- Care Manager experience
- Utilization Review experience
- Utilization Review (InterQual)
- Discharge Planning
Preferred Qualifications
- BSN or MSN from an accredited school of Nursing
Key Responsibilities
- Review work lists to prioritize patients and identify new admissions
- Document assessment and plan of care in Epic
- Participate in daily care management touchpoints
- Attend CAPP meetings and Complex Care Meetings
- Coordinate with Social Work, Utilization Management, and the multidisciplinary team on discharge barriers, psychosocial concerns, and transition planning
- Educate patients and families on community resources and post-discharge needs
- Monitor observation patients throughout the day for progression of care
- Verify patient understanding and agreement with the discharge plan
