RN Care Manager

Overview

• Location: Chapel Hill, North Carolina
• Setting: Acute care hospital
• Shift: Days
• Hours: 40 hours per week
• Contract length: 13 weeks

Role Summary

Provides ongoing patient assessment, care planning, implementation, and evaluation to support case management, utilization review, and discharge planning.

Required Qualifications

• Valid RN license in North Carolina
• Associate of Science in Nursing / Associate Degree in Nursing
• 2 years of Case Management experience
• Case Management experience within the last year
• Experience in an Acute Care or Hospital setting
• Acute Hospital Case Management experience
• Care Manager experience
• Utilization Review experience
• Utilization Review (InterQual)
• Discharge Planning

Preferred Qualifications

• BSN or MSN from an accredited school of Nursing

Key Responsibilities

• Review work lists to prioritize patients and identify new admissions
• Document assessment and plan of care in Epic
• Participate in daily care management touchpoints
• Attend CAPP meetings and Complex Care Meetings
• Coordinate with Social Work, Utilization Management, and the multidisciplinary team on discharge barriers, psychosocial concerns, and transition planning
• Educate patients and families on community resources and post-discharge needs
• Monitor observation patients throughout the day for progression of care
• Verify patient understanding and agreement with the discharge plan
