Description

Working under the direction of the program DPCS and within an interdisciplinary team philosophy, the nurse will w ork directly with physicians in identifying and coordinating patient/family/caregiver home care needs and providing skilled supportive care to patients and their families.  Utilizes the nursing process to provide skilled care and comfort and maximizes the quality of life to the patient, family and caregiver.  Refers and coordinates members of a multi-disciplinary team (other disciplines and specialties) transfers to other levels of care and services with other providers as appropriate.  Case manages patient care within the guidelines of the various payor sources, California state, Joint Commission and company policies.  Prepares patient/family for discharge when appropriate, referring to community resources. Education/Experience/Certifications & Licensure Possession of a clean license to practice as a Registered Nurse in the State of California.  One year of experience as a professional nurse in a medical?surgical facility within the last three years or one to two years of recent home health care experience preferred. Possession of current CPR certification.  Skills Required Must be sufficient in skills listed below: General Dressing changes w/ Aseptic Technique Urinary catheter insertion/maintenance, irrigation, collecting urine samples ??Gastrostomy Tube Care Infusion: PICC dressing change & sterile technique Oxygen Concentrator Venipunctures Colostomy EMR system is MyUnity (NetSmart)  Will be assigned a “base” region within Monterey County, however, may be asked to see patient based on needs anywhere in agency service area. Schedule start/stop time is flexible – normal day is 8-5, M-F with rotating weekends. Performance – complete visits on 4-5 patients average per day to include planning and charting. Willing to train as needed understanding should be independently working within 2 weeks.

Gaudy and Gina are awesome!!!

I worked a while ago for Tact. I have no idea why there are bad reviews but they are amazing!!! Gaudy deserves such a great recognition for her work and placing me in the epicenter of the pandemic and doing her job amazingly! She was also promoted. Gina recruiter is so nice, good , caring and understanding. I love them the company I just wasn’t able to do a travel job again at the time they were helping me again in 2022/3 but they are great to work for and with!
