Description

American Traveler is hiring an experienced RN Case Manager for an inpatient care management position requiring acute hospital discharge planning experience and NJ or compact RN licensure.

Details

• Hospital-based inpatient Case Management/Utilization Review role
• Day shift schedule, 8:00 AM to 4:00 PM, 37.5 hours per week
• Rotating weekends required every 4th or 5th weekend
• Holiday coverage required for one summer and one winter holiday
• Floating as needed, including to a sister hospital nearby
• No on-call responsibilities
• Uses Epic EMR for charting

Requirements

• Active RN license (NJ or Compact) required
• BSN required
• CCMC certification preferred
• Minimum 2 years of acute inpatient hospital discharge planning experience required
• Hospital experience required, including discharge planning and placement in rehabilitation facilities
• Experience with subacute patients required
• In-Patient Case Management experience required
• Experience with Managed Care preferred

Additional Information

• Coordinate patients through acute care hospitalization and manage all aspects of discharge planning and transition process
• Perform utilization management and quality screenings
• Accepts local candidates at a lower rate
