What You'll Do
Deliver skilled nursing care under the direction of a Registered Nurse and physician
Follow and implement individualized Plans of Care
Monitor patient conditions and communicate changes to the RN Case Manager and care team
Educate patients and caregivers on disease management, medications, and self-care techniques
Document assessments, interventions, and patient progress accurately and timely
Promote patient safety and implement strategies to reduce avoidable hospitalizations and emergency room visits
Collaborate with physicians, nurses, and interdisciplinary team members to ensure quality patient outcomes
Support patients and families with compassionate, respectful, and patient-centered care
Identify opportunities for improvement and communicate recommendations to the RN Case Manager
What You'll Bring
Graduate of an accredited Associate Degree Nursing program
Current state LVN/LPN license
Minimum of one (1) year of clinical nursing experience required
Home Health or Hospice experience preferred not required
Strong communication, critical thinking, and patient care skills
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
Willingness to travel within the assigned service area
