Job Summary
Cuready Healthcare Staffing is seeking a qualified RN, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Coordinate and manage patient care delivery during hospital stays and after discharge.
- Conduct comprehensive assessments of patients' medical, functional, and psychosocial needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Facilitate transitions of care, including patient education and follow-up appointments.
- Utilize InterQual Criteria and Milliman Guidelines for care planning and review.
- Collaborate with insurance companies for authorization and coverage verification.
Qualifications
- Bachelor of Science in Nursing (BSN) required.
- Active California Registered Nurse License.
- AHA Basic Life Support (BLS) certification.
- Minimum of three years of acute hospital experience preferred.
- One year of required case manager experience.
- Proficiency with electronic health records and case management software.
