Full-Time | Monday–Friday
East Hampton & Surrounding Area

• Monday–Friday schedule (no weekends required!)

• Deliver skilled nursing care under RN/physician direction

• Follow and implement individualized patient care plans

• Monitor patient conditions and report changes promptly

• Educate patients and families on self-care and safety

• Administer medications and maintain accurate documentation

• Collaborate with RNs and interdisciplinary care teams

• Help prevent avoidable hospitalizations through proactive care

• Active LPN/LVN license (state required)

• Graduate of an accredited nursing program

• 1+ year of clinical nursing experience

• Home Health or Hospice experience (preferred)

• Ability to travel throughout the East Hampton coverage area (~50%)

• Reliable transportation, valid driver’s license, and auto insurance

• Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs
