Essential FunctionsPerforms all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.Ability to formulate an individualized plan of care, revise plan as indicate by the patient's response to treatment, and evaluate overall plan daily for effectiveness.Monitors, records, and communicates patient condition as appropriate to care team, provider, patient, and family following chain of command as appropriate.Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.Works as an advocate for the physical and emotional well-being of the patient protecting the patient's (resident's) rights.Documents patient/resident assessment information appropriately including documentation that is used in completing the Resident Assessment Instrument (Minimum Date Set). </body>
