Job Summary

We are seeking a qualified professional to assist our medical team with discharge planning, assessing Durable Medical Equipment (DME) needs, and providing recommendations for patient follow-up. The ideal candidate will have experience in acute hospital settings and possess a strong ability to facilitate patient care transitions.

Key Responsibilities

• Collaborate with healthcare professionals to develop and implement effective discharge plans for patients.
• Assess patient needs for Durable Medical Equipment (DME) and ensure appropriate procurement and training for use.
• Provide recommendations for follow-up care and community resources to enhance post-discharge recovery.
• Communicate effectively with patients and their families regarding discharge instructions and care coordination.
• Maintain accurate records of discharge plans, patient assessments, and recommendations.
• Participate in interdisciplinary team meetings to discuss patient care strategies and outcomes.

Qualifications

• Must possess a valid Washington State license.
• Experience in inpatient acute hospital settings is required.
• Strong communication and interpersonal skills.
• Knowledge of DME products and resources available for patient care.
• Ability to work collaboratively in a fast-paced environment.
