We are currently seeking two experienced RN Care Managers for a 13-week assignment in a full-time capacity. These positions will have variable shifts, either from 8:30 AM to 4:30 PM or four 10-hour shifts from 7:30 AM to 5:00 PM. While there is no call required, weekend and holiday rotation will be necessary. The RN Care Manager will oversee concurrent utilization management activities across the organization and will serve as the primary link between the patient, healthcare team, organization, and payors throughout the patient's hospital stay.

Duties and Responsibilities:

• Conduct assessments of patient needs and identify resources required for effective transitions to the next care setting.
• Collaborate with and lead an interdisciplinary team during daily care rounds, care conferences, and care planning sessions.
• Utilize excellent communication skills across written, verbal, and electronic formats, including email and applicable software systems.
• Work with community organizations to secure necessary resources, such as durable medical equipment, health insurance authorizations, and transportation arrangements.
• Participate in managing the length of stay for the inpatient population.
• Maintain knowledge of hospital reimbursement methods and applicable quality measures related to the care management department.

Education/Licensure/Certifications/Qualifications:

• Associate or Bachelor’s degree in Nursing with 3-5 years of clinical experience in medical/surgical nursing, home care, or hospital case management.
• Experience with utilization review and discharge planning is required.
• Certification by a nationally recognized case management or utilization review organization is preferred.
• Current BLS certification is required; CCM or ACM certification is preferred.

Shift Information:

• Variable shifts: 8:30 AM - 4:30 PM or four 10-hour shifts from 7:30 AM - 5:00 PM.
• No on-call responsibilities.
• Weekend and holiday rotations are required.

Minimum Years of Experience:

3 years of relevant experience is required.

Certifications / Requirements:

BLS/BCLS certification is required; CCM or ACM certification is preferred.
