Description

American Traveler is seeking an experienced RN for a Float/Home Health position in the South Region of Massachusetts, requiring at least 1 year of certified home care and case management experience with an active MA RN license.

Details

• Home health setting with patient visits conducted in patients' homes across a wide South Region service area, including communities such as Braintree, Quincy, Brockton, Plymouth, Dorchester, and surrounding towns
• Serves a diverse patient population ranging from newborns to patients over 100 years of age
• Day shift hours: 8:00 AM – 4:30 PM
• Weekend rotation required once every 6 weeks
• On-call and holiday coverage required per departmental rotation
• EMR: Epic (charting system used for all documentation)

Requirements

• Active MA RN license required — no pending or conditional licenses accepted
• AHA-issued BLS certification required
• Minimum 1 year of certified home care experience required
• Minimum 1 year of case management experience required
• Reliable, independent means of transportation required for home visits
• Flu vaccine required; no religious or medical exemptions accepted
• 2 professional references required for consideration
• Primary source verification of professional license required within 30 days of start date

Additional Information

• Responsibilities include assessing patient conditions, implementing individualized care plans, and providing patient and family education and support
• Performs admissions processes and case management in accordance with MGB Healthcare at Home policies and standards of practice
• Requires thorough and timely electronic documentation and interdisciplinary communication
• Time and attendance tracked via UKG/Kronos
• Local candidates residing within 60 miles of Massachusetts General Hospital in Boston are eligible to apply
• Parking is not validated at Boston-area locations; public parking is available at candidate expense, with daily rates typically ranging from $17–$22; other service area locations may also have parking costs
• RTO requests must be disclosed at the time of consideration
