Description
American Traveler is seeking an experienced RN for a Heart Transplant Coordinator role requiring at least 2 years of Med/Surg or ICU experience and transplant experience.
Details
- Outpatient/clinic-based setting within a large academic medical center
- Role focuses on coordinating care for heart transplant patients pre- and post-transplantation
- Patient population includes pre-transplant candidates and post-transplant recipients
- Responsibilities involve attending transplant clinics in person on site
- Schedule is Monday–Friday, 8:00 AM–4:30 PM, 3 days per week with flexible scheduling
- Epic EMR experience strongly preferred
- On-call responsibilities include organizing transplants and triaging patient problems by telephone
Requirements
- Active MD or Compact RN license currently in hand
- Current BLS certification required
- Minimum 2 years of recent, strong Med/Surg or ICU nursing experience required
- Transplant experience required
- Experience at a facility with 500+ beds required
- Strong patient triage skills, including the ability to triage simple and complex problems by telephone
- Proficiency with computer data entry and legible documentation required
- SSN and DOB required for consideration
- 1 professional reference required for consideration
Additional Information
- Coordinates pre-transplant preparation by arranging and monitoring required testing, providing patient education, and ensuring internal and external activities are aligned
- Manages post-transplant recipients including prescription refills, medication reconciliation, chart reviews, and clinic coordination
- Collaborates with a multidisciplinary transplant team including pharmacy, social work, and nutrition
- Local candidates are accepted at the same rate as travelers
- Heart transplant coordination experience is strongly preferred
