Description
American Traveler is seeking an experienced RN Case Manager with at least 3 years of experience for a Monday–Friday day shift position in a hospital case management setting requiring a GA or compact RN license.
Details
- Hospital-based Case Management unit
- Day shift schedule, Monday–Friday, 8:00 AM–4:30 PM (5x8-hour shifts)
- No weekend or on-call requirements
- Floating within the facility and scope of practice as needed
- 12-week contract length
Requirements
- Active GA or compact RN license required
- Minimum 3 years of RN Case Management experience required
- Two supervisory references (Manager level or above; charge nurse references not accepted) are highly preferred
- Copies of all relevant certifications required for consideration
Additional Information
- Responsibilities include assessing, evaluating, and driving discharge planning; coordinating services to the appropriate level of care; ensuring treatment plans are initiated and re-evaluated daily; monitoring length of stay; identifying barriers to discharge; and following Medicare regulatory guidelines including patient choice, IMM notices, and MOONs
- First-time travelers are not eligible for this position
- Local candidates are accepted
- RTO is capped at 7 days; RTO cannot be requested during the first week of the assignment
- If one or two major holidays fall within the assignment period, travelers are expected to work all of them with no holiday RTO allowed; if three major holidays fall within the assignment period, travelers are expected to work two, with one holiday RTO request eligible for review
- Major holidays include: Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Year's Eve/New Year's Day
- Travelers must have been away from all facilities within this health system for a minimum of 1 full year before returning as a traveler
- No special scheduling requests, alternate shifts, or schedule changes are permitted — the posted shift is firm
- Travel pairs may not be assigned to the same shift on the same unit
