Description

American Traveler is hiring an experienced RN Case Manager for a home health position requiring home health case management experience and OASIS proficiency.

Details

• Home health setting serving an adult patient population
• Case management role with documentation responsibilities
• Day shift schedule, 5x8-hour shifts (8:00 AM – 4:30 PM)
• Floating may be requested based on facility needs
• Mileage reimbursed at the IRS rate
• OASIS charting system experience required

Requirements

• Active RN license — VA license in hand or proof of compact license required
• Current BLS certification (must be AHA-issued)
• Minimum 1 year of home health case management experience required, within the last year
• Proficiency with OASIS documentation required
• SSN required for consideration
• One supervisory reference required for consideration

Additional Information

• Responsibilities include adult home health case management and clinical documentation
• Local candidates are accepted at the same rate
• Prior permanent employees must have been separated for at least 6 months before being eligible
• RTO should be kept to a minimum — no full calendar weeks off preferred, and no more than 7 days total; all RTO must be submitted upfront and no additional RTO requests will be approved after an offer is made
