Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health and OASIS experience for a day-shift travel assignment.

Details

• Home health setting providing in-home physical therapy services
• Day shift schedule, 5x8 hour shifts from 8:00 AM to 4:30 PM
• 13-week assignment length
• Epic experience required for charting
• Floating may be requested based on facility needs
• Local candidates accepted at the same rate

Requirements

• Minimum 1 year of home health experience required
• Proficiency in OASIS documentation required
• Current BLS certification required
• Manual Therapy certification preferred
• Specialty experience within the last year required
• License required if applicable
• Epic charting experience required
• Skilled in patient assessment, gait training, and use of assistive/adaptive devices
• Experience with orthopedic, neurologic, and cardiopulmonary patient populations preferred
• SSN required at time of consideration
• One supervisory reference required for consideration

Additional Information

• Responsibilities include home exercise program development, patient evaluations, and ongoing reassessments
• Documentation duties include OASIS, plan of care, long and short term goals, and discharge summaries
• May involve ordering supplies and scheduling patient appointments
• Business casual attire or solid-colored scrubs permitted
• Candidates cannot work Riverside travel and per diem contracts simultaneously; must choose one
• Prior permanent employees must have a 6-month gap before returning as a traveler
• RTO should be minimal, ideally no full calendar weeks off and no more than 7 days total, requested at time of initial submission
• No additional RTO requests approved after an offer is extended
