Description
American Traveler is hiring an experienced RN for a Case Management/Care Coordination position in an acute care hospital setting, requiring prior CA Dignity/CommonSpirit experience and proficiency with Cerner and Midas.
Details
- Acute care hospital setting, Care Coordination (Case Management) unit
- Patient assignment ratio of 1:20
- 8-hour day shifts, every other weekend required
- First to float; must be able to cover all adult areas of the hospital (travelers without women's health experience will not be assigned to cover women's health)
- Charting systems include Cerner, Midas, InterCall, and Curaspan
- EMR/system proficiency in Cerner, Midas, InterCall, Curaspan, and Microsoft Office is required
- 8-week assignment
Requirements
- Active CA RN license required; must be active at time of consideration — pending licenses are not accepted
- Active BLS certification required; pending certifications are not accepted
- Minimum of 3 years of hospital-based RN experience required
- Acute level care coordination experience required
- Prior experience at a CA Dignity Health or CommonSpirit facility is required for consideration
- Proficiency with Midas, InterCall, Cerner, Curaspan, and Microsoft Office required
- Experience with discharge planning and utilization review required
- Copy of a valid driver's license required for consideration
- 2 professional references required: one supervisor from within the last 12 months worked and one peer or supervisor from within the last 3 years worked, both including dates of employment and eligibility for rehire
Additional Information
- Responsibilities include discharge planning, utilization review, and care coordination for a socially complex patient population including uninsured/underinsured individuals, conservatorship cases, and board-and-care placements
- Familiarity with placement agencies, Medicare/Medi-Cal guidelines as they relate to SNF and lower-level placement, and strategies for discharging under-resourced patients is strongly preferred
- Orientation consists of 4 days of webinar-based charting training followed by 2 days with a preceptor
- RNs must pass the Relias Dysrhythmia exam (basic or advanced) before an offer can be confirmed
- A pass/fail skills test on sterile technique for Foley catheter insertion and central line dressing changes is administered on the first day of orientation
- Business casual, conservative dress required; no open-toed shoes; lab coats worn; minimal to no perfume
- Candidates who were directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity facility within the past year are not eligible; candidates employed PRN through outside staffing firms will be considered
- Minimal time-off requests preferred for this assignment
- Pharmacy and respiratory therapy support available 24/7
- Must be a fast learner, adaptable, and able to hit the floor running; excellent attendance is expected
