Description
American Traveler is seeking an experienced RN for a Case Management/Care Coordination position at an acute care facility in Sacramento, CA, requiring prior CA Dignity/CommonSpirit acute care experience and proficiency with Cerner and Midas.
Details
- Acute care hospital setting in a Care Coordination (Case Management) unit
- Patient assignment ratio of 1:20
- 8-hour day shifts with every other weekend (EOW) required
- First to float; must be able to cover all adult areas of the hospital (travelers without women's health experience will not be assigned to cover that area)
- Charting system: Cerner (experience required); must also be proficient with Midas, InterCall, Curaspan, and Microsoft Office
- 13-week contract
Requirements
- Active CA RN license required (must be active at time of consideration; pending licenses not accepted)
- Current BLS certification required (pending certifications not accepted)
- Minimum of 3 years of hospital experience with acute-level care coordination experience required
- Prior experience at a CA Dignity Health or CommonSpirit acute care facility is required
- Proficiency with Midas, InterCall, Cerner, Curaspan, and Microsoft Office required
- Experience with discharge planning and utilization review required
- A copy of a valid driver's license is required for consideration
- Two professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked (both must include dates of employment and eligibility for rehire)
- BSN not required
Additional Information
- This unit serves a heavily underserved area of Sacramento; candidates should have a strong background in socially complex discharge planning, including placements such as room and board, board and care, and conservatorship
- Familiarity with placement agencies, Medicare/Medi-Cal guidelines as they relate to SNF and lower-level placement, and strategies for discharging under- or uninsured patients is strongly preferred
- Orientation consists of 4 days of webinar-based charting training followed by 2 days with a preceptor
- Candidates must pass the Relias Dysrhythmia exam (basic or advanced) before an offer can be confirmed
- A pass/fail sterile technique test (Foley catheter insertion and central line dressing changes) is administered on the first day of orientation
- Business casual, conservative dress code required; no open-toed shoes, lab coats worn, and minimal to no perfume
- Candidates who have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity-affiliated facility within the past year are not eligible; candidates employed PRN through outside staffing firms will be considered
- Must be a fast learner who is adaptable and able to hit the ground running; excellent attendance is expected
