Description

American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week home health assignment in Kingman, AZ.

Details

Home health setting serving patients in the Kingman, AZ area
Patient diagnoses include CVA, heart failure, Type II diabetes, post-amputation, Parkinson's disease, paraplegia, and dementia
Monday through Friday day shift, 8:00 AM – 4:30 PM (5x8-hour shifts)
No on-call requirements

Requirements

Minimum 1 year of physical therapy experience required
Home health or outpatient PT experience required
Current AZ PT license required (must be in hand at time of consideration)
Current BLS certification required
Level 1 fingerprint card required
Two professional references required for consideration
PDF copies of driver's license and Social Security card required for consideration

Additional Information

Responsibilities include evaluation and treatment of home health patients across a variety of neurological, cardiac, and musculoskeletal diagnoses
