Description
American Traveler is seeking an experienced RN Case Manager with home health case management experience for a 13-week travel assignment requiring a CA RN license.
Details
- PACE (Program of All-inclusive Care for the Elderly) care setting
- Case Manager role responsible for an assigned patient caseload
- Day shift schedule, 5x8-hour shifts (8:00 AM – 4:30 PM)
- 13-week contract
Requirements
- Active CA RN license required
- Current BLS certification required
- Home health case management experience required
- Minimum 2 years of RN experience required
- Two supervisor references obtained within the past year required for consideration
- Candidates must reside more than 50 miles from the facility to qualify for travel pay rates
Additional Information
- Responsible for overall patient management including direct care delivery, care planning, and coordination of the interdisciplinary team
- Manages visit utilization and expense control within the assigned caseload
- May assist with agency-wide orientation, competency evaluations, and clinical staff consultation
- Former employees or travelers must have been separated from this health system for at least 1 year before returning as a traveler
- Only one holiday RTO request will be approved during the contract
