Description
American Traveler is hiring a Case Management RN for a day shift position requiring 2+ years of case management experience and EPIC EMR proficiency.
Details
- Works in the Case Management department of a 495-bed Level II Trauma Center hospital
- Unit has 20-30 beds with patient ratios averaging 1:20-1:30
- Patient population includes Medicare, Medicaid, Commercial, and Unfunded cases
- EPIC EMR experience is required
- 5x8 day shift schedule from 8:00 AM to 4:30 PM
- Every other weekend work is required
- Potential on-call responsibilities
- Floating required within the Care Management department
Requirements
- Active VA RN license or Compact license required
- BLS certification from AHA required
- 2+ years of RN Case Management experience required
- No certifications expiring within the first 30 days of assignment start
Additional Information
- Coordinate care management activities for assigned patient caseload
- Facility has earned an 'A' grade in Patient Safety for 9 consecutive years
- May request one holiday off, with a maximum of two days holiday RTO considered
- Business casual or eggplant purple scrubs required per facility dress code policy
- Candidates must reside within a 50-mile radius of the facility
- Candidates must be well qualified with all required experience
