Description
American Traveler is seeking a Physical Therapist with 1+ years of experience for a post-acute setting covering both SNF and Home Health.
Details
- Post-acute care setting covering both Skilled Nursing Facility (SNF) and Home Health (HH), split approximately 50/50 week to week
- Primarily based in Grand Island with potential need to assist with PTO coverage in Hastings or Kearney areas
- Day shift schedule, 8-hour shifts, Monday-Friday or shifted schedule (Tuesday-Saturday or Sunday-Thursday) with day lengths that may slightly vary
- 40 hours per week guaranteed for 8-10 hour shifts
- Will supervise up to 2 Physical Therapist Assistants
- Uses Net Health and HCHB charting systems
- Productivity expectations: 82% for SNF (evaluation, group, and concurrent time counted in full) and 95% for HH (includes drive time and documentation time)
- No call or call-back requirements
Requirements
- Minimum 1 year of experience required
- Current BLS certification required
- Valid driver's license required for home health travel between locations
- License required if applicable
- Two professional references required: one supervisor from within the last 12 months worked and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire
Additional Information
- Responsibilities include evaluating and treating patients across both SNF and HH settings
- Onsite therapy supervisor, clinical specialist, and director available for support and training
- New graduates will be considered and trained
- Mileage and drive time are billable between locations and between patients during home health visits
- Work week runs Sunday through Saturday
- Local candidates accepted at an adjusted rate
- 80-mile radius rule applies for local placement determination
