Description
American Traveler is hiring an experienced RN Case Manager for a hospital setting requiring 2 years of acute Case Management experience and an active MD or compact RN license.
Details
- Work in the Case Management department of an acute care hospital
- Manage a patient ratio of approximately 1:18
- Day shift schedule, 5x8 hour shifts, Monday through Friday from 8:00 AM to 4:30 PM
- Uses Cerner charting system
- 13-week contract assignment
Requirements
- Active RN license (MD or Compact) currently in hand
- Current BLS certification
- Minimum 2 years of recent Case Management experience within the last 7 years required
- Acute rehabilitation, discharge planning, interdisciplinary collaboration, and care coordination experience required
- Acute hospital experience required
- Experience with Cerner charting system required
- Teaching hospital and community hospital experience preferred
- Prior travel assignment experience required for candidates without recent Case Management experience on a med/surg unit
- Two supervisory references required, including one from the most recent assignment, supporting at least 2 years of experience within the previous 3 years
Additional Information
- Perform care coordination, discharge planning, and utilization review for patients on medical/surgical units
- Conduct needs assessments, order DME, and determine medical necessity per evidence-based guidelines
- Collaborate with interdisciplinary teams and refer patients to appropriate disciplines, rehab, or long-term care facilities
- Document patient care, plan of care, and coverage criteria in accordance with regulatory standards
- Facilitate discharge and contingency planning, including knowledge of community-based service programs
- No RTO permitted during the first two weeks of the assignment, with a maximum of 7 days RTO per 13-week contract
- Candidates with previous employment at this health system must have been separated for at least one year before returning as a traveler
- Local candidates residing within 60 miles are accepted at a lower pay rate
- First-time travelers are not considered unless they have relevant recent hospital Case Management experience on a med/surg unit
