Description

American Traveler is hiring an experienced RN Case Manager for a hospital setting requiring 2 years of acute Case Management experience and an active MD or compact RN license.

Details

• Work in the Case Management department of an acute care hospital
• Manage a patient ratio of approximately 1:18
• Day shift schedule, 5x8 hour shifts, Monday through Friday from 8:00 AM to 4:30 PM
• Uses Cerner charting system
• 13-week contract assignment

Requirements

• Active RN license (MD or Compact) currently in hand
• Current BLS certification
• Minimum 2 years of recent Case Management experience within the last 7 years required
• Acute rehabilitation, discharge planning, interdisciplinary collaboration, and care coordination experience required
• Acute hospital experience required
• Experience with Cerner charting system required
• Teaching hospital and community hospital experience preferred
• Prior travel assignment experience required for candidates without recent Case Management experience on a med/surg unit
• Two supervisory references required, including one from the most recent assignment, supporting at least 2 years of experience within the previous 3 years

Additional Information

• Perform care coordination, discharge planning, and utilization review for patients on medical/surgical units
• Conduct needs assessments, order DME, and determine medical necessity per evidence-based guidelines
• Collaborate with interdisciplinary teams and refer patients to appropriate disciplines, rehab, or long-term care facilities
• Document patient care, plan of care, and coverage criteria in accordance with regulatory standards
• Facilitate discharge and contingency planning, including knowledge of community-based service programs
• No RTO permitted during the first two weeks of the assignment, with a maximum of 7 days RTO per 13-week contract
• Candidates with previous employment at this health system must have been separated for at least one year before returning as a traveler
• Local candidates residing within 60 miles are accepted at a lower pay rate
• First-time travelers are not considered unless they have relevant recent hospital Case Management experience on a med/surg unit
