Description
American Traveler is hiring an experienced RN Case Manager for a 13-week Days contract requiring 5+ years of hospital case management experience and a ME or compact RN license.
Details
- Care Management department in a 32-bed acute hospital unit
- Patient ratio of 1:20-24
- High acuity, complex patient population with a high percentage of behavioral health cases
- Day shift schedule, Monday through Friday, 7:30 AM to 4:00 PM
- Every third weekend and one holiday required
- Floating required to cover other Care Management department needs as needed
- Uses EPIC, Wellsky, Careport/PASSR, Teams, laptop, and Ascom phone
Requirements
- Active RN license (ME or compact) required upon consideration
- Minimum 5 years of hospital case management experience required
- Minimum 5 years of hospital acute adult care nursing experience required
- Discharge planning and care coordination experience required
- EPIC EMR experience required
- Driver's license required for consideration
- 2 professional references required
Additional Information
- Responsible for discharge planning, care coordination, and case management for high acuity, complex patients
- Ideal candidate is flexible, easygoing, calm under pressure, and a strong problem solver
- Business casual dress code; scrubs or professional casual attire acceptable
- Float policy applies within a 25-mile radius between facilities as needed
- Candidates residing within 90 miles of any MaineHealth facility (Maine or New Hampshire residents) are considered local, with a reduced bill rate
- Local travelers are not eligible for renewals or extensions
- RTO should be minimal, with 3-4 days considered reasonable for the 13-week contract
- First-time travelers may be considered
