Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of experience and AZ PT licensure.

Details

• Home health physical therapy setting with outpatient exposure
• Patient diagnoses include CVA, heart failure, DMII, s/p amputation, Parkinson's, paraplegia, and dementia
• Schedule is Monday through Friday, day shift (4x10 hours, 7:00-17:30)
• No on-call requirements
• Daily travel expected, typically 30-75 miles but up to 150 miles in some cases, with mileage reimbursed at IRS standard rate

Requirements

• Minimum 1 year of experience required
• Home health or outpatient PT experience required
• Current AZ PT license required in hand at time of consideration (license required if applicable)
• BLS certification required
• Level one fingerprint card required
• Meditech charting experience highly preferred

Additional Information

• Responsibilities include assessment, evaluation, treatment planning, and documentation across a range of neurologic, orthopedic, and cardiopulmonary conditions
• Holiday coverage is required and RTO should be kept minimal
• Travel pairs accepted
• First-time travelers accepted
• Former permanent employees must wait 12 months before returning as travelers
• Modules included as part of orientation and are non-billable
