Description

American Traveler is hiring an RN Case Manager for an acute care hospital, requiring a NJ or compact RN license and prior inpatient discharge planning experience.

Details

• Hospital-based Care Management unit
• Coordinates patients through acute care hospitalization
• Manages all aspects of discharge planning and transition process
• Performs utilization management and quality screenings
• Works with subacute patients and discharge placement to rehabilitation facilities
• AM shift, 8:00 AM to 4:00 PM, 37.5 hours per week
• Rotating weekends required every 4th or 5th weekend
• One summer and one winter holiday coverage required
• May float to a sister hospital as needed
• No on-call requirements
• Uses EPIC for charting

Requirements

• Active NJ or compact RN license required
• BSN required
• CCMC certification preferred
• Minimum 2 years of acute inpatient hospital discharge planning experience required
• Hospital experience required, including discharge planning and placement in rehabilitation facilities
• Experience with subacute patients required
• Case Management and Managed Care experience required
• In-Patient Case Management experience required

• Coordinates comprehensive discharge planning and transition of care for acute hospital patients
• Performs utilization review and quality screening as part of daily responsibilities
• Local candidates accepted
• Travelers may extend but are limited to one calendar year on assignment before requiring a break
