Description
American Traveler is hiring a Home Health RN Float requiring an active MA RN license and at least 1 year of certified home health and case management experience.
Details
- Home health setting covering the Lynn, Nahant, and Saugus areas within the North Region
- 13-week assignment with an 08:00-16:30 shift schedule
- Weekend rotation required once every 6 weeks
- Ability to travel to patient homes as assigned
- Uses Epic for electronic charting and documentation
Requirements
- Active MA RN license required, no pending or conditional licenses accepted
- Minimum 1 year of experience specifically within a certified home health agency
- Minimum 1 year of case management experience within a certified home health agency setting
- AHA-issued certifications only
- Flu vaccine required with no religious or medical exemptions allowed
- Primary source verification of license required through both NURSYS and BON
Additional Information
- Responsible for assessing patient conditions, implementing care plans, and educating patients and families in the home setting
- Performs admission processes and case management duties per organizational policies and standards
- Requires thorough and timely electronic documentation and interdisciplinary communication
- Care delivered in accordance with community health practice standards and organizational philosophy
- Local candidates residing within 60 miles of Massachusetts General Hospital in Boston will be considered at a reduced rate
- Uses UKG/Kronos for timekeeping
