Position Overview

• Job Category: Clinical – Nursing (Local Contract)

• Role: Licensed Vocational Nurse (LVN)

• Setting: Home Health

• Number of Openings: 2

• Patient Population: Young adults to geriatrics

• Reporting: Works under direction of RN, physician, Site Supervisor, or Site Manager

Core Responsibilities

• Provide patient care within the scope of the Vocational Nurse Practice Act

• Assist with assessment, planning, implementation, and evaluation of patient care

• Implement and document individualized discharge plans

• Collect, handle, and process specimens appropriately

• Perform complete and accurate patient charting

• Maintain accurate equipment and quality measure records

• Assist with care and maintenance of department equipment

• Collaborate with interdisciplinary care teams across the continuum

• Ensure a safe, ethical, and HIPAA-compliant care environment

• Recognize ethical, legal, and medical dilemmas and escalate appropriately

• Participate in continuous quality improvement initiatives

Required Experience (HARD STOP)

• Minimum 1 year of Home Health experience REQUIRED

• At least 1 year of experience in a healthcare facility

• Specialty clinics may consider new grad LVNs (not applicable for this Home Health role)

Required Licensure & Certifications

• Active California LVN License

• BLS Certification (current)

Preferred Certifications

• Phlebotomy Certification

Required Skills & Competencies

• Basic keyboarding skills (minimum 35 WPM)

• Proficiency with Microsoft Office (Outlook, Word, Excel)

• Strong communication and interpersonal skills

• Customer-focused mindset

• Strong attention to detail and organizational skills

• Ability to prioritize tasks and meet deadlines

• Ability to work effectively in a multidisciplinary team

• Knowledge of infection control, safety measures, and proper body mechanics

• Experience with Electronic Medical Records (EMR)

• Knowledge of medical terminology

• Knowledge of patient scheduling, registration, and insurance verification

• Order entry systems experience
