Registered Nurse – Case Manager

Facility:  Doctors Community Medical Center

Duration: 13 weeks
Shift: Day 5x8-Hour (08:30 - 17:00)
Schedule: Monday through Friday with rotating weekends and holidays
Charting System: Epic
Float Required: Yes – within scope
Note: Schedule may be 08:00-16:30 or 08:30-17:00.

Position Overview:

Provide comprehensive case management and care coordination services, collaborating with patients and interdisciplinary teams to support appropriate care delivery, discharge planning, and continuity of care.

Job duties:

• Assess patient needs and coordinate individualized plans of care in collaboration with the interdisciplinary healthcare team.
• Coordinate transitions of care, including discharge planning, referrals, and follow-up services.
• Monitor patient progress and communicate changes in clinical and psychosocial needs to appropriate providers.
• Facilitate communication among patients, families, physicians, nursing staff, and community resources to support continuity of care.
• Document case management activities, patient assessments, care plans, and coordination efforts accurately in the electronic medical record.

Requirements:
License: Active Maryland RN or Compact RN license required
Certifications: BLS
