Description

Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)

Job Summary

  • Provide home health nursing care including patient assessment, care plan implementation, and ongoing education/support for patients and families.
  • Perform admission processes and case management duties following organizational policies and community health standards.
  • Deliver patient care consistent with organizational philosophy and standards.
  • Utilize thorough and timely electronic documentation and interdisciplinary communication.
  • Ability to travel to assigned cases.
  • Weekend rotations occur approximately once every 6 weeks.

Experience Requirements

  • Minimum 1 year of Certified Home Care experience.
  • Minimum 1 year of Case Management experience.

Contract Length

  • Approximately 13 weeks.

Credential & Compliance Requirements

Required for Submission:

  • License/Certificate Verification (Primary source verification of nursing license; MA Conditional Licenses not accepted).
  • Resume including past 7 years of work experience with city and state details.
  • Two recent references (minimum one supervisor or above in skillset).
  • Skills Checklist.
  • Candidate Submission Coversheet.

Required for Onboarding:

  • Color photo (taken within past 6 months; no selfies, filters, eyeglasses; plain background).
  • Motor Vehicle Record (MVR) check within 45 days of start.
  • Fit Test for N95 respirator within 365 days of start, updated annually.
  • Nurse Aide Registry check within 30 days of start (required by MA DPH for all employees).
  • Documentation of MMR vaccination or proof of immunity.
  • Influenza vaccine during flu season (Sept 15 - April 30), no exemptions allowed.
  • TB test documentation within 3 months of start (skin test or blood test), updated if >6 month break.
  • Physician Health Statement within 365 days of start, updated if >6 month break.
  • Criminal Background Check within 45 days prior to start, updated if >6 month break.
  • Massachusetts CORI background check within 30 days prior to start, updated if >6 month break.
  • Respirator Medical Clearance Attestation (part of Fit Test record).
  • Confirmation of Assignment/POI form for initial assignments and extensions.
  • Education verification through background check company (highest level achieved).
  • Social Security Number (SSN) Trace with criminal check.
  • National Sex Offender Registry check within 30 days of start, updated annually.
  • Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually.

Optional / Preferred:

  • Hepatitis B vaccination or proof of immunity or declination.
  • Tdap vaccine within 10 years (preferred).
  • Positive PPD with negative chest X-ray documentation if applicable.
  • TB symptom survey if positive PPD (updated annually).

Additional Notes

  • No religious or medical exemptions allowed for N95 fit testing or influenza vaccine.
  • Fit testing cannot be performed onsite; must be completed prior to start.
  • Background checks and credential verifications are strictly enforced and must be current.
  • Compliance with all MA Department of Public Health regulations is mandatory.

This position is ideal for experienced home health RNs with case management skills who are comfortable traveling to patient homes and working within a structured, policy-driven environment.
