Job Opportunity: RN Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Required Experience & Qualifications
- Minimum 1 year Certified Home Care experience
- Minimum 1 year Case Management experience
- Current licensure in the state of Massachusetts
- Reliable, independent transportation for home visits
- Willingness to work weekends, holidays, and be on call as needed
- Weekend rotations occur once every 6 weeks
Job Responsibilities
- Provide clinical services to patients ranging from newborns to over 100 years old in their homes
- Assess patient conditions and implement care plans
- Provide ongoing education and support to patients and their families
- Perform admissions and case management duties according to organizational policies
- Maintain thorough and timely electronic documentation and interdisciplinary communication
Prior Travel Experience
- No explicit requirement for prior travel experience
Licensure & Certifications
- Must hold a valid Massachusetts nursing license (MA Conditional Licenses not accepted)
- Primary source verification of license required (NURSYS and Board of Nursing verification)
Health & Safety Requirements
- N95 Fit Test within 365 days of start date, updated annually (no religious exemptions)
- Documentation of MMR vaccination or immunity
- Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions
- TB test documentation within 3 months of start date (skin test or blood test), updated if >6 month break
- Hepatitis B vaccination strongly preferred (3 doses or immunity proof)
- Tdap vaccine preferred (within 10 years)
- Physician health statement within 365 days of start date, updated if >6 month break
- Respirator Medical Clearance attestation if fit test required
Background & Screening Requirements
- Criminal background check within 45 days prior to start, updated if >6 month break
- Massachusetts CORI background check within 30 days prior to start, updated if >6 month break
- Social Security Number trace with criminal check
- National sex offender registry check within 30 days prior to start, updated annually
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start, updated annually
- Motor Vehicle Record (MVR) check within 45 days of start (required for all home care positions)
- Nurse Aide Registry check within 30 days of start (required by MA Department of Public Health for all employees)
Documentation & Submission Requirements
- Resume including past 7 years of work experience with city and state
- Two recent references (minimum one supervisor or above in skillset)
- Skills checklist
- Candidate submission coversheet
- Confirmation of assignment/POI form upon acceptance and for extensions
Additional Notes
- Weekend rotations are scheduled once every 6 weeks
- Must be willing to work on-call, weekends, and holidays as per department needs
- Reliable transportation is essential for home visits
This position is ideal for an experienced RN with home care and case management background, licensed in Massachusetts, who is comfortable working independently in patients’ homes and flexible with scheduling.
