Description
Registered Nurse - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 17 weeks
Job Summary
Provide clinical nursing services to patients of all ages in their homes. Responsibilities include assessing patient conditions, implementing care plans, and offering ongoing education and support to patients and families. Perform admissions and case management duties following established policies and standards, with thorough electronic documentation and interdisciplinary communication.
Must be willing to work weekends, holidays, and be on call according to department rotation (weekend rotation approximately once every 6 weeks). Reliable, independent transportation is required for home visits.
Required Qualifications
- Valid Registered Nurse (RN) license in Massachusetts
- Minimum 1 year of Home Health experience within the last year
- Minimum 1 year of Case Management experience
- Experience in a Home Care setting
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Required Credentials & Compliance (must be completed prior to or at onboarding)
- Professional license verification (primary source) within 30 days of start
- Criminal background check, including Massachusetts CORI and national sex offender registry
- Motor Vehicle Record check (MVR) for driving positions
- Health screenings and immunizations:
- MMR (measles, mumps, rubella) vaccination or proof of immunity
- TB test within 3 months of start (skin test or blood test) and updated if break >6 months
- Hepatitis B vaccination or proof of immunity (strongly preferred)
- Fit test for N95 respirator within 1 year of start and updated annually
- Health statement signed by a licensed provider within 1 year
- Education verification of highest degree achieved
- References: 2 professional references within last 4 years (minimum 1 supervisor-level)
- Skills checklist completed within 1 year of submission
- Submission of a professional photo meeting specified criteria
- Additional background checks including SSN trace, exclusion lists, and nurse aide registry check
Additional Information
- Must be able to work independently in a home care environment
- Documentation and communication skills required for electronic health records and interdisciplinary collaboration
- Weekend rotations occur approximately once every 6 weeks
- On-call availability required per department needs
