Description
Job Opportunity: Registered Nurse - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (approximately 13 weeks)
Job Summary
Provide clinical nursing services to patients of all ages in their homes. Responsibilities include assessing patient conditions, implementing care plans, and offering ongoing education and support to patients and their families. Perform admissions and case management duties following established policies and standards, with thorough electronic documentation and interdisciplinary communication.
Weekend rotations occur approximately once every 6 weeks. Willingness to work weekends, holidays, and be on call as needed is required. Reliable, independent transportation is necessary to make home visits.
Required Qualifications
- Valid RN license in Massachusetts
- Associate of Science in Nursing / Associate Degree in Nursing
- Minimum 1 year of home health nursing experience within the last year
- Minimum 1 year of case management experience
- Experience in a home care setting
Required Onboarding and Compliance Credentials
- Professional license verification (primary source, within 30 days of start)
- Criminal background check, including state and national registries
- Motor vehicle record check (for driving to patient homes)
- Tuberculosis screening (skin test or blood test within 3 months, updated if break >6 months)
- Measles, mumps, rubella (MMR) vaccination or immunity documentation
- Health statement signed by a licensed provider within 1 year
- Fit testing for N95 respirator within 1 year, with medical clearance attestation
- Documentation of immunizations and health screenings as required (e.g., Hepatitis B preferred, Tdap preferred)
- Resume including last 7 years of work experience
- Two professional references (minimum one supervisor-level) within last 4 years
- Skills checklist completed within 1 year
- Education verification (highest level achieved)
- CORI (Massachusetts criminal offender record information) check
- Sex offender registry check
- Exclusion list checks (OIG, GSA, Medicaid exclusions)
- Nurse Aide Registry check (required by MA Department of Public Health for all employees)
Additional Information
- Must be willing to comply with all onboarding and credentialing requirements prior to start
- Weekend rotations approximately every 6 weeks
- Independent travel to patient homes required
This position is ideal for an experienced RN with home health and case management expertise who is comfortable working independently in a community setting.
