Job Details

• Weekly Estimated Pay: $2,120 - $2,230
• Assignment Duration: 13 weeks (May 25, 2026 – August 21, 2026)
• Hours: 40 hours per week (Monday–Friday, 8:00am–4:30pm)
• Shift: 8-hour shifts
• On Call/Weekend: One weekend per month
• Coverage Area: Louisville Metro, Kentucky (home health visits across Louisville)
• Dress Code: Scrubs (any color)
• Travel: Mileage reimbursed between visits (from first patient to last patient)

Qualifications / Job Requirements

• Active Registered Nurse (RN) license in Kentucky
• Associate Degree in Nursing (ADN) minimum; BSN preferred
• Minimum 1 year of home health nursing experience using HCHB (Homecare Homebase)
• Professional certification required; OASIS certification is a plus
• Strong clinical skills in IV therapy, wound care, and congestive heart failure (CHF) management
• Excellent documentation and time-management skills for OASIS and electronic charting

Responsibilities

• Provide skilled nursing care in patients' homes per physician orders and individualized care plans
• Conduct Start of Care visits, ongoing assessments, and complete accurate clinical documentation in HCHB
• Complete timely OASIS assessments and meet productivity expectations (30 points per week)
• Coordinate interdisciplinary care with case managers, therapists, and primary care providers to optimize outcomes
• Perform IV therapy, wound assessments and treatments, medication management, and CHF monitoring with a focus on patient education
• Deliver compassionate, patient-centered care that supports independence and safe transitions from hospital to home
