Job Details
- Weekly Estimated Pay: $2,120 - $2,230
- Assignment Duration: 13 weeks (May 25, 2026 – August 21, 2026)
- Hours: 40 hours per week (Monday–Friday, 8:00am–4:30pm)
- Shift: 8-hour shifts
- On Call/Weekend: One weekend per month
- Coverage Area: Louisville Metro, Kentucky (home health visits across Louisville)
- Dress Code: Scrubs (any color)
- Travel: Mileage reimbursed between visits (from first patient to last patient)
Qualifications / Job Requirements
- Active Registered Nurse (RN) license in Kentucky
- Associate Degree in Nursing (ADN) minimum; BSN preferred
- Minimum 1 year of home health nursing experience using HCHB (Homecare Homebase)
- Professional certification required; OASIS certification is a plus
- Strong clinical skills in IV therapy, wound care, and congestive heart failure (CHF) management
- Excellent documentation and time-management skills for OASIS and electronic charting
Responsibilities
- Provide skilled nursing care in patients' homes per physician orders and individualized care plans
- Conduct Start of Care visits, ongoing assessments, and complete accurate clinical documentation in HCHB
- Complete timely OASIS assessments and meet productivity expectations (30 points per week)
- Coordinate interdisciplinary care with case managers, therapists, and primary care providers to optimize outcomes
- Perform IV therapy, wound assessments and treatments, medication management, and CHF monitoring with a focus on patient education
- Deliver compassionate, patient-centered care that supports independence and safe transitions from hospital to home
